Hi Colin,
You mention injectors were blocked with swarf? sounds to me like your high pressure pump is breaking up, swarf in tank contaminating injectors & fuel lines, if this is the case the only fix is to replace injectors, pump , pipes and a thorough clean of fuel lines and tank , maybe even replace tank if not 100%.
Fuel pressure & leak off tests i would look at if it was me
