Years ago, there was a bad one near Watford. My dad had worked on signaling side since early 1960s . He told me they would blame the driver, but the real reason was lack of investment on telecommunications. Sure enough, that was what happened.
They did clear the driver years later. Very similar to the helicopter incident between N.Ireland and Scotland. Still more to come out there though. Although maybe not in our lifetime.
