totally understandable. I mainly work in cardiac surgery, major vascular surgery (AAAs), Trauma, major abdominal surgery (Whipples and liver tansplantation) so I guess (from my view point) I only see the adult IV induction side of it. Ive NEVER seen a gaseous induction for an adult. So far, Ive only been with an anesthesiologist twice when we did peds. The reason they used Sevo (probably while most do this for peds) was because they could then get the patient asleep (place the ET tube), and then while on Desflourane, could insert IVs to manage fluids. Kids are scared enough as it is having to go to surgery, the last thing they need is to be poked and prodded more than needed-especially when its totally reasonable to do it after induction. Point well taken.
