I have had that problem and also trying to get around it, giving mivacurium at the end to just get a few minutes more relaxation only to find it gave me a lot more than expected. What I have done as a result is when I have to redose, I give 25% of my original dose and see what happens with the PNS. But it does seem there still occasions when reversal is difficult. Consider the "OLD" anesthetist trick on a belly case, turn off the ventilator and hand bag. When the surgeon goes to stitch, let go of the bag so you are not pushing the abdominal contents up. After the stick is in and they are pulling up on the suture, bag the patient. Sometimes timing the ventilations in tune with the work, you can get away with not having to give more relaxation. (I call this an "old" anesthetist trick because along with hand masking a patient, bagging seems to be a skill that is not used that much anymore. Everyone gets paralyzed, everyone gets stuck on the ventilator.)
