It is not like people get into anesthesia without having some idea of how the system works. I do not believe any AA or CRNA could be that naive. If you are an AA or CRNA you must realize that if you work for MDAs, you will be in the room during the entire case (unless they give you a break?) and they will be covering the rooms either from the hallway, the break room or the lounge. To enter into the "relationship" knowing the arrangement and then turn around and say the MDAs are stealing "your money" is a bit lame.

And FYI, it is not just MDAs that make money off of other anesthesia providers. I am aware of several CRNAs groups in various states where you have a few "senior partners" who do all the billing and collections, then they hire other CRNAs to work for the group for a fixed salary. They may tell the new hires that after a certain period of time they can become "partners" also but it never seems to happen so the salaried CRNAs get mad and leave, only to be replaced by new salaried CRNAs who get the same promise. The "senior partners" make a fortune off the backs of the salaried CRNAs, who for some reason also seem to do the majority of the cases.

Be wary of any employment position where you get a fixed salary. The work load will only increase and you will not see an proportional increase in compensation for your extra work. Take a fixed salary for 40 hours per week, and an hourly amount for anything more than 8 hours a day or 40 hours per week. Also be wary of places that give "comp time" for over time. They usually will not let you save it up for additional vacation time but with rather make you take it back in little 1-2 hour blocks on days that wind down early.
