Any chamber adapter will lower the pressure of the load used. Some of the information given here is a bit superfluous to your question, especially the 20 to 28 barrel lining. You should be aware that Federal now manufactures a 12 gauge "Top Gun" loading of 7/8 ounce at around 1200 fps. This load would be very similar to a 20 gauge target load fired in a chamber insert. Give it a shot before you spend a bunch of money on chamber inserts. To answer your last question, "Yes, you can fire about any 2 3/4" 20 gauge load in a chamber insert with safety." However, your initial post indicated that you were looking for a very light load because of a health situation.
