As you probably already realize the was caused by a leaking injector seal.
Upon removing the injector a large deep groove was visible in the injector seat, caused by the escaping compression gasses.
Under normal circumstances this writes off the cylinder head and as far as the customer was concerned in this case the van.
We agreed to try a fix and cut the seat down until the groove was gone, then made a bespoke insert to bring the injector back to the correct height and be able to seal again.
So far the customer has done several hundred miles without the seal failing again
