What's funny is that I have to think that if you grew up in the 60s or maybe even early 70s that anchovies were a very regular topping. So much that it was almost included without asking. Why else as kids would we have heard so many time "Get a large pizza and someone would yell out AND NO ANCHOVIES!"

Another thing. I think some point in the early 80s american finally learned how to eat pizza. Because of the biggest problems with pizza was it always burned the roof of your mouth. I even remember a scene in the movie The Sure Thing where he was talking about writing a paper on the topic how to eat pizza without burning the roof of your mouth.
