    William, for the safety to not work when the gun is open would mean that the safety push rod is missing out of the slot.

    As for the rotary bolt closing when the barrels are open, sounds as if the spring on the bottom of the trip lever is broken or missing.

    This sometimes happens when a gunsmith does not know how an L.C. Smith looks like inside.
