Has anyone else received a "vehicle insurance verification form" from the Secretary of State?

It asks you if you had insurance as of (date listed on form).

You mark yes, and provide the Insurance company name and policy number, or you mark no, with an explanation as to why you do not have car insurance.

It gives you 30 days to return the form, or have your license suspended.

I haven't received one yet, but, I know 2 people that received one within the last couple of weeks.
