I had a similar situation with my OB when I needed my FMLA paperwork filled out for work saying I was indeed pregnant and would need time off. I got a bill in the mail for $25 for filling out the paperwork.
It wouldn't have been so bad if I hadn't had to call the office 3 times to make sure it was getting filled out and faxed in a timely manner. Then when it didn't arrive the day after I was required to have it there (thus pushing back the day my maternity leave was starting) I had to call back and have it sent again. Apparently, the fax number was "too small" for the nurse to read on this standard government form, so on Friday the 15th she dropped paperwork in the mail mailed saying my maternity leave started on Sunday the 17th. Smart.
