Money - simple as that. It takes money to start a program. Faculty, staff, facilities, equipment. AA programs are tied to medical schools and anesthesia residencies, so the financial well-being of those departments is a factor as well. Those departments are often hard-hit by reimbursement issues and federal statutes that treat anesthesiology differently than other specialties. Attempts to remedy this situation has been opposed by (GASP) CRNA's and the AANA.
UMKC is ramping up now. At least one or two more are in the planning stages, and a couple more are in the discussion stage.
