Be very careful of "supervision" that is only on paper. I personally know of one situation where an MDA was billing for "supervising" and not once saw the patient pre-op, intra-op, or post-op and caught the chart at the end of the day and signed off on it as if he was involved. That happened so often it got reported and he AND THE CRNA (and probably this would also apply to an AA) both got charged with Medicare fraud, insurance fraud, falsification of documents and etc. The MDA because he did it, the CRNA because he knew it was being done and did nothing to stop it. Granted it was ONE MDA in over 30 years of experience, but I bet "supervision on paper" happens more often than we like to admit.
