Has anyone seen this before ? and know what caused it? Cylinger 3 inlet valve is burned and as a result of escaping exhaust gasses has melted the inlet manifold, the piston has also started to melt. The glow plug is also melted.

Hello Andrew, a few things I can think of that might cause a cylinder to overheat. E.G.R valve not seating / blocked exhaust, Faulty injector, Glow plugs not shutting off, Excessive D.P.F re-gens. Does the egr pipe go to the manifold nearest no. 3 cylinder?
