Skoda Octavia 1.9 TDi BJB 2005 - Lack of power and smoking, turbo boken - Fixed #

Year of Manufacture *: 2005
Engine Size and Code *: 1.9 bjb

I have a skoda octavia that came in suffering with lack of power and smoking.
there was oil in the mouth of the turbo
I took the turbo off to see if there was any obvious signs of damage, and yes there was. the shaft had broken and the propeler on the manifold was broke of and stuck in the turbo.
What I am asking is, is any reasons this could happen, is there a problem before the problem, so when i replace the turbo it doesn't happen again. Is it common.
There is oil in all the intercooler pipes and emulsified oil inside the intercooler, like scum. Whats the best way to clean out intercooler and pipes.

Check all pressure pipes ,these can split & leak causing turbo overspin,also have the sump off & check the oil pickup they dont need much of an obstruction to kill the turbo,make sure intercooler & pipework are spotless otherwise it could run on its own oil hth keith.

I don't want to sound like an amatuer, but when you say all pressure pipes which pipes are you on about.

I have almost finished the job now. I Changed the turbo, degreased the intercooler and pipe work, blowing all water out. When I start the engine I am going to leave the pipe that connects to the throttle body off for a couple of minutes, to let any fluid hidding to leave. I took the Sump off. there was nothing out of the ordinary. That should be it. I'll clear the codes and see how she is. I hope I havn't made any rookie mistakes
