I would expect cold start problems too if it was air ingress, but have you checked filter housing, might be worth changing the filter with new seals. Also I've seen the fuel pipes rub through under the air filter box if they've not been clipped down properly.

Might also be worth checking the fuel pressure sensor with a scope, see if it's building pressure as it should.
Also an injector leak off test.
