A shot in the dark here, forgetting about your codes for a moment, has the vehicle had a new aux drive belt recently? we have seen a few of these engines with the camshaft pulley locating peg sheared when part of a failed aux belt has got tangled up in the timing case, everything looks ok externally until you start to dismantle a bit.
