Assume this is a D4D engine. Common problem is SCV valves on back of high-pressure pump. Toyota sell a repair kit containing both valves for about £200.From memory one is green , one red , held in with 5mm allen bolts , about 0.5hr to change , no need to remove pump from vehicle.
Pumps can also give trouble but SCVs more likely.
