You could likely exchange the implants to a larger size with added profile. That may or may not improve the nipple position but more implant under the breast would fill out the overall envelope more. Your situation is one of the quandaries with implant reconstruction and nipple sparing mastectomy. With flaps, we can shift the nipple as high as we want once things are healed, like a standard breast lift.....but with implants, we don't have any living tissue under the nipple to support it. So addition implant volume or lowering the implant down on the chest wall to center the nipple over the implant are our primary moves to optimize things.
