I had a bilateral mastrctomy with direct to implant over the muscle about 5 weeks ago and from the beginning i felt my nipples were placed a bit too low (one slightly more than the other). I have heard of implants "settling" and how that could "raise" the nipples upward, but I have not seen any change yet and am worried that this is my final outcome. I was told by the surgeon that I was a good candidate for direct to implant. I had breastfed all of my kids and there was some deflation involved prior to the surgery. I understand my nipples may not have been centered prior to the surgery but I also was told that I would look "augmented" with more upper pole fullness and minimal scarring (infamammary fold). I used to wear 30E bra (which my ps could not believe because i lost significant volume after pregnancies and breastfeeding as well as being small framed). I was told that I am too skinny for any other option but implants. I have some rippling so I think fat grafting was mentioned at my 3 week post op (though he casually mentioned that he wasn't sure where he could get fat from me (97 lbs 5'2). I wondering what my options are if the nipple positions do not change and I want to have a revision, and is there an option to change out the implants too for bigger size/higher profile)? I currently have 355 cc mentor moderate profile (anatomical gummy bear). I am interested in what your advice would be, and if the revision surgery is something I can travel to get done later.
