PAP doesn't appeal to me because I'm not a fan of removing fat from areas that may cause weight bearing problems with sitting or other activities long after the breast reconstruction is done. There are other places to borrow fat that don't carry that potential "expense" to long term well being. Other than that, healing problems and seromas are sometimes mentioned as downsides to the PAP as well as a less than hoped for donor site look.

You'd probably be better served by simply having a bilateral upper hip flap if you have enough there (most do, even if tummy thin).
