Glad you are doing awesome! The insurance company doesn't know anything about breast monitoring as you found out, they just regurgitate what their computer says when they enter a request.
This would be best handled thru our breast surgical oncologists so that they can advise re. letter/phonecall/rewrite of order and so forth. Give the office a call, tell them you "spoke" with me and you need to discuss this with Dr. Stolier or Lazarus so that they can help you out.
