Perhaps disparities in healthcare could be another reason...
IF SA has a poorer system there could be more deaths to otherwise treatable conditions that could mean there aren't as many people in the vulnerable category who could fall prey to Covid.
but demographic profile seems to be a significant one. Where I am (Oman) has 3.7% 65+ and 74 deaths per 100,000 which seems inline with the SA/UK trend. Compliance has been pretty poor.
