Curious. In Sheldon Leonard's autobiography he prints an example of a script from TAGS. It's the 'moody gypsy' sequence from the 'Opie Loves Helen' episode, Season 5, 1964. He credited the script with a random name chosen out of the LA phone book.
Sheldon states it was a constant battle for, not script approval, but writer approval. Each name he had to submit to an organization headed by actors Ward Bond and Adolph Menjou, and it got so tiresome he eventually used blacklisted writers and just picked a random name to show as writer credit. At least all the way into season 5 it was still a problem.
