Not cocking when opened

I have a L.C. Smith Feild model and when I open it, it will not cock. I can take it apart and use a wrench to manually cock it, but once I shoot it with snap caps and open it, it won't cock. I've tried taking it apart and manually recocking then putting it back together like this site says to do. Is there anything I could be doing wrong when putting it back together that would cause it not to cock?

Both locks fail to cock? Is the cocking plate in the forend in place?

The cocking plate is missing. Could it have fallen out when cleaning it or is it pretty secured to the forend?

Not uncommon for the screw to shear.
