If all things are equal since it fired correctly. ie. no one has taken the stock off or over tightened any screws or removed the floor plate , then it sounds like you have a worn sear. The recoil from firing the first bbl is causing the second (worn) sear to loose its hold on the hammer and discharging the second bbl
