This story goes way back to the early 1960s. I bought the Pyro plastic model kit of the USS Maine, on which box cover was an accurate illustration of the ship whose destruction started the Spanish-American War. When I opened up the box and began building the model, I was astonished to see that it was, in fact, a rather poor representation of the cruiser USS Olympia (I had already built the Revell model of this ship), with an added third turret in center line amidships! I eventually filled the completed model with firecrackers and blew it up during a fourth of July celebration.
For myself I would have to say was the purchase of Polly Scale WW2 5-H Haze Gray. Instead of the WW2 pigment all 4 bottles contained modern USN Haze Gray. Very strange indeed but useful. I guess even Polly Scale slipped in quality control.
