I can't think of a reason why this particular piece would have been inherited by her nieces and nephews. As it did not appear on an auction it may have been bought from her estate and in that scenario the most likely buyer was a Belgian Royal and probably the current king.
As for reason B. it was a gift of Spain to Fabiola when she became Queen of the Belgians. That it was under the Franco regime does not alter the fact that it was a gift of the Queen's home nation and should be honoured as such.
I could see a scenario where Mathilde will not wear the piece until she has worn it in Spain like Maxima waited for the State Visit to the UK to wear the Stuart diamond.
If the Spanish Royals have no issues with wearing jewels given to them by/during the Franco regime i see no reason why the Belgian Royals would need to act differently.
