I have a couple question regarding the letters of recommendations. First are the evaluation forms the same thing as a letter of recommendation or do we need to have the person fill out the evaluation form and attach a separate letter with it?
Secondly, do your reference send the forms and letter directly to the school? Or can they fax or scan it and send it to you and then you send it to the school once you've finished your application?
Lastly, who are the best people to get letters from? teachers/ employers/ aa's? Would you advise asking one of the AA's that you shadowed for a couple of hours to fill out the form as well?
