That’s a great start for information Susan! You’ll also need your Dad’s birth certificate! And you might need your own cuz somehow you’ll have to prove how you are related to your Dad, and that would be your Dad’s name on your birth certificate!!! But the process is kinda long but you’ll get the records! You can call 312-534-4440 and they will send you or let you know how to get the application and let you know what else you’ll need!!! We like to tell people to be sure to put in a note about how you would like ALL of his records….which is different than some people who are requesting records of someone who went to a Catholic school which was not a residence place like Angel Guardian! You’ll want all religious records and school records and medical records and living records which would be things about how he lived in the cottage he was in! They will call you after you send in the application to let you know how many pages you will be asking for and let you know the price of the records! That’s when you send in your payment and then they’ll send you the records or you could pick them up!! Of course that’s how it was before Covid….so hopefully it’s the same now!!!
