Your charger (they are not common!) is lovely. I might guess that it dates from the later 19th century. We don't (yet) have this pattern in our data base.
The only reference on Minton in my library is Priestman's "An Illustrated Guide to Minton Printed Pottery, 1796-1836", and I don't see your pattern there. Kowalsky's Encyclopedia of Marks references an impressed "Mintons" mark, dating 1873-1951. Further, the swan (again, Kowalsky) is identified as a year cipher, ranging from 1895 to 1900, as you state. The heart is probably a workman's mark. "51" may be, also, but perhaps was meant to be "15", which would be the diameter of the piece (?).
Thanks for your submittal. Hopefully someone out there has a reference which can provide a pattern name.
