You as an employee are required to provide medical facts to your employer. Your doctor is providing the information to you as a patient/paying customer. What you do with your information from has nothing to do with the Dr. You can follow there advice if you want but trust me; you won't like the outcome.
I don't know why people make this so hard. You're at the Dr. office. Get the HCPS. Problem solved.
