SJ..I don't think that's industry standard. My friend works for Aetna and makes those types of decisions and she's surprised to hear this. She says sometimes the dr will write things up that will cause the insurance company an upcharge but a good representative will contact the physician and make the correction to get the patient what is needed. She's willing to talk to you if you want her to. Just let me know.
