I was looking for a simmilar device when I started experimenting with smoking as a teen, in fact, my first experiment involved a ballpoint pen casing, a coffee Jar, and a length of Aquarium tubing, it was quite amazing to see the volume of undiluted smoke that was created, and fill the jar, but, it didn't taste quite right, so I figured that something was going on, and desisted with that line of expermentation. It wasn't till a few years later that I read information that supported my theory, at least in part, and research confirmed it.
Since you're set on a holder, and I don't know of a company that would market such a device, you have one of two options, you can browse a Lab. supplies company and see what lengths and diameters of glass tubing they have available, just make sure when you make a purchase that the inside diameter is equal to or slightly greater than the external diameter of the cigarette, as you can always create a roughly airtight seal with a thin strip of paper or adhesive tape. The other alternative is contacting a glassblower, and supplying them with the specifications of what you want done, it might cost significantly more, but, it'll be a lot closer to what you want.
