My back-up plan includes the NICU at the hospital that is literally two blocks away from my house. If something goes wrong, transferring will be quite easy.
I haven't read "When You're Expecting Twins, Triplets and More" but my midwife specifically recommended "Having Twins and More" by Elizabeth Noble. She also gives great advice, but is a bit more supportive of natural birthing choices.
