My twins are only 18 months now, so that's not yet an issue, although they still manage to get us to get them out of their cribs!
I've seen crib tents that are mesh tents that you put over the crib to preven climbing out. I'm not sure how I feel about that myself, but if I were desperate enough, I might try it.
I've also read that you can put a gate up in the door to the room to treat the room like a giant crib. You can put a pillow and blankets near the doorway, so that if they fall asleep there, they're comfy. I read this in Healthy Sleep Habits, Happy Child which has been a great resource for me.
