It is Infanta Isabel Alfonsa of the Two Sicilies.
She was the daughter of Don Carlos Bourbon T-S and his first wife Infanta Maria de las Mercedes, elder sister of Alfonso XIII.
Isabel Alfonsa was closely related to the SRF, as she was the first cousin of the Count of Barcelona (via her mother) and the half-sister of the Countess of Barcelona, (via her father) and, therefore, a half-aunt (does that exist ? 
) of Prince Juan Carlos .
