Hi Ryan!
One other thing, because you don't mention it. Are you using a full grip or a thumbless grip? Since the bar is pressed into your back you don't need to wrap your thumb around the bar. I used to have the same problems you are describing and when I switched to a thumbless grip my wrist pain went away and it was eaiser to grip closer to my shoulders, keep my elbows back, and pull the bar into my back to keep it tight.
