Dear Jeannette,
Small implants are generally the way to go and if that approach is used it is rare to have a problem. If the implants had to be removed, the breast would flatten from the loss of the volume and options for correction would include replacement with another implant later or addition of a latissimus flap under the existing flap vs. accepting the flattened affect on the breast.
