Dear Cari,
A Tram or DIEP can be used to fill expanded skin and sometimes that will allow avoidance of a skin paddle addition. Better to have immediate reconstruction with a tissue reconstruction and skip the expander step altogether. Be very sure you research the difference between Tram and DIEP before you commit as the loss of your abdominal muscle on both sides is irreversible and you are very young.
