I am so sorry that you have had to go thru all of this. I often wish I had a magic wand that I could wave over women that have had experiences like your and start all over from the beginning.
There is a fair chance that the permanent implant will be uncomfortable but it should be less painful than the expander. If it does, then you can have the implant out and a GAP flap replace it.
Re. the nipple, you should have an oversized nipple reconstructed perhaps with an associated dermal graft and even consideration of reduction of the opposing nipple.
