Dear Barb,
It depends on which would give you a better result based on the amount of breast tissue you have and the overall shape of your breast with respect to choice of over or under. Remember, under the muscle tends to move in odd ways like your reconstructed breast does/will do with chest muscle contraction. If your gynecologist has concern for imaging, may be best to consider MRI annually as a compliment to mammogram.
