All reconstructed nipples flatten out to a degree over time. The amount depends primarily on the thickness of your skin and the size of the nipple as originally constructed. So making the nipple larger helps when possible. Very few (1 in 10 years) of my patients request a very prominent nipple, so most are pleased that things shrink.
I only recommend lipo when I think it will help and the patient wants it. Some don't care to have any lipo done and that is perfectly ok. The fat doesn't move around after lipo or grow in areas that wouldn't have become fat with weight gain, its just the liposuctioned areas don't gain as much weight as they did before the procedure. So, what you have heard is untrue.
