Thank you Dr. Dellacroce! Just a few more questions, now that I am on a roll: What breast shapes decides the lower side vs. under the breast incisions or inframammary location? Now for nipple viability, you mentioned approx. a week after surgery would be the crucial "wait & see" period. What happens if the nipple is lost? Can the areolar be salvaged and a new nipple created? Or would the entire NAC need to be removed? What type of intra-op nipple biopsy is done to determine if there are any atypical/ca cells within the nipple? Have you experienced any nipples removed at the time of surgery for this reason?
How many nipple losses have you seen in the past 4 yrs. or since the time DIEP/NS was started? Can you share why these might have happened?
Is there a miniscule amt of tissue left under the nipple to maintain preservation? I know the nipple is cored, but it seems there would have to be some tissue left for viability. Correct?
