Yes, we have been performing nipple sparing DIEP for about 4 years now and have done tow or three hundred or so. The results ,when done well, can be well worth the extra effort. Most women choose to have additional improvement of their potential outcome with a second stage dedicated to making the result everything it has the potential to be but it is not mandatory. Those who don't need the breast shape improved or don't want the benefit of touchup in the donor site, can be single staged. This isn't new just not frequently chosen by those women seeking the absolute best outcomes.
The biopsy will not likely affect the success of the operation. The health of the nipple will be apparent within the first week after surgery, not during the surgery itself. The mastectomy incision would either be on the lower side of the breast or under it depending on your breast shape.
