I'm not a huge fan of lat flaps. Big scar across the back and loss of one of the biggest muscles in your body. Furthermore, you still have to have an implant and even with a lat flap, the previous radiation can cause problems with the implant because the overlying skin is usually still tight. If you're thin, you should consider evaluation for GAP flap reconstruction before you commit to lat flaps. Be sure you look carefully into your options and understand everything about the donor site, the possible result, and the long term expectancies related to the reconstruction.
