do you have a corporation with tax ID number for him? Do you have a W-9 with that information on it to give to him? Do you have Liability insurance in your business name for $1 million and $3 million aggregate? Do you have workman's comp policy to present to him? Do you have professional liability insurance $1 mill with $3 mill aggregate. I assuming you gave him a complete contract. His legal team will require all of these things and if they don't you should have them on your own for your protection. Remember you are still personally liable for the work you do and cannot hide behind a corporate shield. Is he allowed to go over the Arizona Hospital Associations head? If you don't have all these things I would be happy to put the contract in our name and pay you pre-tax and give you 3% of gross of every nurse who works under that contract for the life of the contract.
