1) "Our" and "we" is traditional (I'm sure there's a more detailed explanation, but I don't know it). The governments write the throne speeches, so they probably have their reasons.
2) The last time Parliament was dissolved was last September, so it probably happened then.
3) The Prime Minister has no actual authority over the Governor General other than as an advisor, so it would have to be the Queen (I believe there was a press release from the Martin government saying that the Queen had approved the extension of Adrienne Clarkson's "term"). It's probably less than formal, though, as the Governor General has no "term" to be extended.
4) Whatever the Prime Minister is prepared to tell the Queen when he faxes over the letters patent.
