The best thing to do is to take it into Photoshop and resize it so that it's 720x480 pixels. If it's an odd shape to begin with, start by making the "image size" one of those dimensions, and don't worry about the other. Then make the "canvas" size the 720x480. Save it as a .tga, and import into Premiere. Then use the Project>Utilities>Batch Processing to convert it to an .avi.
