The transplant center should provide you with a list of requirements. Mine required a complete dental workup and a letter from my dentist. (I had some questionable wisdom teeth that weren't causing any trouble then, but they decided to get rid of them before transplant instead of taking a chance that it would be needed later.)
I cant comment on the issue of moles unless a dermatologist thinks that they might develop into a precancerous state once you are immunosuppressed. I think that this is very hard to predict and it would be more a matter of watching them closely after transplant unless you have had skin cancers removed previously.
